Purpose of Survey: The purpose of this survey is to better understand how our products are performing for the people we want to help. Collecting survey data could demonstrate our product effectiveness, highlight customer wants and product strengths, and ultimately inform our formulation so that we can improve our supplements.

Who Can Participate: Anyone 18 years or older who has taken or is planning to take ProbioFerm probiotic capsules.Procedure: In order to participate customers will be asked to complete a short introductory survey, Plan to take ProbioFerm probiotic capsules at least somewhat regularly for at least 30 days and complete a short follow-up survey.

Probiotic Information: Probiotics are live bacteria and yeasts that have beneficial effects on your body. These species already live in your body, along with many others. Probiotic supplements can add to your existing supply of friendly microbes. Anonymity: The data collected through the anonymous surveys will be used to observe trends, strengths, and weaknesses. No information you share will be able to be tracked to you. The two surveys will be used for comparison. The surveys themselves will be deleted once the raw data has been transferred to a secure file. No personal identification will be linked to this data.

Voluntary nature of the Trial: Your participation is voluntary, and you may refuse to participate or withdraw at any time without penalty. You may also refuse to answer certain questions if you wish. The willful giving of any personal testimony is completely voluntary and not an expectation.
